HTML/CSS it is clean and not confusing. The responsive sites coding has become so cumbersome, you have to give up graphics so it can load at the same speed HTML did several years ago.

Lately I have been working on mainly small businesses with new websites or giving them face lifts. I am also spending a great deal of time doing SEO and digital marketing plans. I have a very large client with members of the team globally located. That makes for interesting Skype meetings.
I usually give my clients a to do list at our first meeting. If they do not have a domain and/or hosting server, I will walk them through it right then and there.
However if someone is not sure about what they want or need, I suggest they visit my website and on the home page is a list to help them decide what they want done. On the website development page there is a list of what they will need to provide me at the beginning, and during the development.
Not really an issue. I realize their greatest problem is communicating what they want. I try to learn about their business and place myself in their shoes. This way when I present work, it is a good starting point for both of us.
No, I use excel to keep track of clients and projects. For proposals, I use an invoice template in the Google apps. I do send out status reports or emails, depends on the client and what they would like to receive and how often.
